New patients that
live within the
practice boundary
may register with the practice.
You are required to complete the practice registration form either for an adult or for a child if under the age of 16.
The practice
requires two forms of
identification including one that
demonstrates that you live within
the practice area and one piece of
photographic ID that shows you are
entitled to access NHS services free
of charge. For example, a British or
European passport, European driving
licence or a passport with an
appropriate visa. You will not be
registered with the practice until a
member of staff has seen the
identification described along with
the practice registration form.
When you register with Montpelier
surgery, you register with the
practice, not a specific doctor. You
may however request to see a doctor
regularly and in fact we would
encourage you to do so as you will
get better continuity of care, but
we cannot guarantee that your
request will be fulfilled on every
occasion.
